A flexible service that tackles those small (but vital) problems which prevent patients from being discharged safely

What we do:

Provide discharge-ready clothing, shoes, & 3 days of food essentials

Fit key-safes, handrails, move furniture

Troubleshoot issues like: heating, hot water, lights, fridge, trip hazards, alarms

Fund assessments for deep cleaning or de-cluttering

Order mobility equipment

Alert friends/neighbours when someone’s home

Make welfare calls in the first 3 days after discharge

Expert referrals and signposting to other local services – If we don’t do it, we know who does.
