Connect Well

Online Self-Care, Self-Referral Tool

Do you want to feel more involved in your community, meet new people or make some changes to improve your health and wellbeing?

Then why not let Connect Well help make the first step towards making change a little easier.

Connect Well is a collaboration of mid Essex CVSs: Chelmsford Centre Supporting Voluntary Action, Community 360 Braintree and Maldon & District CVS.

Through the use of trained advisers called ‘champions’ and an online referral system called Connect Well creates an easy way to access a wide variety of support and services on your doorstep.

It can help signpost and connect you to activities including:

• Volunteering opportunities
• Befriending and social activities
• Support and self help groups
• Physical activities
• Managing your money
• Community transport

How does Connect Well work?

Connect Well Champions (trained advisors) are located in venues like council and housing offices, leisure centres, pharmacies and GP practices.

The champions use an online referral system, called Connect Well Essex, to search and signpost you to local groups, activities or networks.

Live Well Link Well

We all want to live well but sometimes we need help to do this.

Social prescribing is ‘more than just medicine’ – it’s a new way to help people find support with their social, emotional and practical wellbeing.

A Citizens Advice report, published in May 2015, suggested nearly a fifth of GPs consultation time was spent dealing with non-medical issues.

Throughout the UK, social prescribing projects are transforming wellbeing in the community and helping to ease pressure on traditional health and social care services.

Live Well Link Well  is our local social prescribing project for people in the districts of Braintree and Maldon and the City of Chelmsford.

It has been funded by Essex County Council and developed with the help of a wide range of health, social care and voluntary organisations.
